  1. *HMOs and PPOs often require a referral or pre-authorization before registration, diagnosis, therapy and medical treatment. Failure of the patient or their referring HCP (Health Care Provider) to provide proper referral or pre-authorization will result in a denial of payment by insurance. Prime Wave Health requires prepayment in full before starting all therapy and treatment plans unless otherwise agreed to in writing.
  2. Prime Wave Health Opt-out providers do not accept Medicare at all and have signed an agreement to be excluded from the Medicare program. This means they can charge whatever they want for services but must follow certain rules to do so.

Medicare will not pay for care you receive from an opt-out provider (except in emergencies). 

  1. You are responsible for the entire cost of your care.
  2. Your provider will give you a private contract describing their charges and confirming that you understand you are responsible for the full cost of your care and that Medicare will not reimburse you.
  3. Opt-out providers do not bill Medicare for services you receive. You are solely responsible for any and all services received while electively receiving therapy, care, and all other products or services available at Prime Wave Health.
